If you're taking Performax
Performax is an as-needed sublingual tablet, meaning you take it only when you plan on being sexually active — not on a daily schedule.
How to take it:
Place 1 tablet under your tongue and let it dissolve. Do not swallow it whole.
Take it 30–60 minutes before sexual activity.
It can be taken with or without food.
Don't take more than one tablet in a 24-hour period.
What to expect: Because it's absorbed sublingually rather than swallowed, many men notice effects faster than with a standard pill — often within 15 to 30 minutes. Some report mild side effects like headache, flushing, dizziness, nausea, or nasal congestion. These are usually temporary. If an erection lasts longer than 4 hours, seek medical attention right away.
If you're taking Tadalafix
Tadalafix is a daily sublingual tablet designed to keep a steady level of medication in your system, so you're not planning doses around sexual activity.
How to take it:
Dissolve 1 tablet under the tongue once daily, at approximately the same time each day.
It can be taken with or without food.
Don't take more than one tablet per day.
What to expect: Because Tadalafix works by maintaining consistent levels in your body, results can build gradually. Some men notice a difference within a few days, while for others it may take a few weeks of consistent daily use to see the full effect. Common side effects can include headache, flushing, upset stomach, back pain, or nasal congestion. As with Performax, seek medical attention if an erection lasts longer than 4 hours.
If you're taking Xtend
Xtend is an as-needed tablet for premature ejaculation, combining an SSRI, a low dose of tadalafil, and L-theanine. It's available in two strengths — Medium and Max — so your provider can dial in the dose that works best for you.
How to take it:
Dissolve the tablet under your tongue or between your cheek and gum. Do not swallow it whole.
Medium Strength: dissolve ½ tablet at least 1 hour before sexual activity — ideally 2–3 hours ahead for the best effect.
Max Strength: start with ½ tablet dissolved at least 1 hour (ideally 2–3 hours) before activity. If it's well tolerated, your provider may have you increase to a full tablet for future doses.
Don't exceed 1 tablet in a 24-hour period, regardless of strength.
What to expect: Xtend is designed to work on a few fronts at once — helping delay ejaculation, supporting erectile confidence, and easing performance-related anxiety. Because it contains paroxetine (an SSRI), it's not something you start and stop casually — don't discontinue it without talking to your provider first. Common side effects can include nausea, drowsiness, dry mouth, sexual side effects, headache, flushing, or indigestion. Xtend should not be combined with another SSRI, SNRI, or serotonergic medication, or used within 14 days of an MAOI, due to the risk of serotonin syndrome. Seek immediate medical attention for signs of serotonin syndrome (agitation, high fever, tremor, rapid heartbeat), any new or worsening suicidal thoughts, or an erection lasting longer than 4 hours.
Important notes:
Consistency matters. Whether it's daily dosing or timing a dose before activity, taking it as directed gives you the best chance of seeing results.
Alcohol in moderation is generally fine, but drinking heavily can increase the risk of side effects like dizziness or low blood pressure, and may reduce effectiveness.
Tell your provider about other medications you're taking, especially nitrates, alpha-blockers, or blood pressure medications — these can interact.
